
Diabetes Program
For newly diagnosed, poorly controlled and interested patients with diabetes
We offer a collaborative diabetes education visit involving the patient (and
caregiver), nurse educator and dietitian which addresses the following
self-care behaviors:
• Healthy eating
• Being active
• Monitoring
• Taking medications
• Problem solving
• Reducing Risk
• Communication with the health care provider
Individuals will be offered:
• Initial individualized consultation session with a nurse and a registered
dietitian. We will provide feedback to your care provider after the education session
has been completed.
• A Follow-up visit with the dietitian (depending on individual needs)
If interested, ask your care provider to refer you for this service.
Coverage for this service varies by insurance company. Please verify your
